This investigation assessed the surgical outcomes and potential complications of employing fibrin glue with double bipedicle conjunctival flaps (FADCOF), a novel operative approach to restore a stable ocular surface in individuals experiencing severe, sight-threatening ocular surface conditions alongside a shortage of bulbar conjunctiva. Painful, blinding ocular surface disease afflicted six eyes belonging to six patients, all of whom were enrolled in this study. Due to prior surgical interventions or ocular surface ailments, all patients exhibited insufficient superior or inferior conjunctiva tissue to completely cover the corneal surface. The period from 2009 to 2019 encompassed the FADCOF treatment for these patients. Surgical success, VAS pain scores, ocular inflammation levels, and post-operative complications were all key results. The surgical procedure was deemed successful if the initial ocular symptoms subsided completely and the ocular surface remained stable, free from flap-related issues like melting, retraction, or dehiscence, preventing any corneal re-exposure. The surgical procedures on all six eyes (100% success) were successful. Every patient undergoing the surgical procedure indicated substantial betterment in their subjective symptoms and the full alleviation of ocular pain (VAS pain score decreasing from 65.05 pre-operatively to 0.00 one month post-procedure). The ocular inflammation score underwent a significant reduction, plummeting from a presurgical score of 183,069 to 33,047 one month subsequent to the surgical intervention. No postoperative complications were detected during the prolonged observation period, lasting from 12 to 82 months. In instances of painful, blinding ocular surface diseases, where a single total corneal flap procedure is not appropriate, FADCOF presents a dependable alternative treatment option. Deep anterior lamellar keratoplasty (DALK) surgery necessitates a keen understanding and accurate measurement of stromal dissection depth. Intraoperative optical coherence tomography (iOCT) for Descemet's Stripping Automated Lamellar Keratoplasty (DALK) faces a key impediment in the form of visualization impairment caused by artifacts generated from the use of metallic surgical tools. A novel surgical technique using suture-assisted iOCT guidance provides clear visualization of corneal dissection planes during DALK. With the aid of a Fogla probe, a stromal dissection tunnel is created, and its depth is then established using a 1 cm section of 8-0 nylon suture, which is passed through the tunnel. The Fogla probe's lack of prominence is in contrast with the iOCT's clear emphasis on the 8-0 nylon. Should the initial tunnel depth prove inadequate, a secondary, deeper stromal tunnel, visualized by iOCT, can be established using an 8-0 nylon suture. This iterative approach allows for a comprehensive dissection of the stroma, thus enhancing the probability of successful big-bubble creation and achieving Descemet's membrane exposure in DALK procedures. A successful big-bubble DALK procedure was performed on a patient with severe keratoconus, employing this particular technique.
Eye injuries from alkali substances require rapid evaluation and therapy to protect vision. Significant alkali-induced ocular damage may result in long-term, vision-threatening consequences such as symblepharon, corneal sores, corneal fibrosis, limbal stem cell deficiency, dryness of the eyes, scar tissue formation in the eyelids and surrounding areas, glaucoma, inflammation of the uvea, and irreversible loss of vision. In order to restore the ocular surface, treatment targets pH neutralization, inflammation control, and regeneration. Significant corneal and conjunctival epithelial damage occurred in a 35-year-old male following direct ocular exposure to sodium hydroxide, despite aggressive initial medical management. A substantial amniotic membrane (AM), externally sutured and augmented with a bespoke symblepharon ring, was applied to the patient post-procedure to promote tissue repair. At four months post-injury, the patient's vision, initially impaired by corneal and conjunctival damage, had improved to a clear 20/25. A teenage girl's ring infiltrate Klebsiella keratitis was the subject of this study, highlighting a singular case. An episode of fever and rash, preceded by decreased vision in the right eye of a 16-year-old girl, was also associated with a burning sensation during urination. Having received proper consent, the patient was examined. SIS17 HDAC inhibitor A ring-shaped corneal infiltrate was seen in her right eye, along with an epithelial defect, on slit-lamp examination. Gram-negative rods, subsequently identified as extended-spectrum beta-lactamase-producing Klebsiella pneumoniae colonies via culture, were found in corneal scrapings subjected to microbiological testing. Fortified amikacin and tobramycin, applied topically, resulted in a positive response from the patient. Due to her systemic complaints, a comprehensive investigation performed by the pediatrician included a blood culture, which exhibited the growth of K. pneumoniae. Consequently, intravenous antibiotics were administered in accordance with the antibiogram results, and the patient experienced a recovery. Subsequent to a period of two weeks, the left eye exhibited a paracentral infiltrate, which was subsequently accompanied by anterior uveitis. Combining topical steroids with aminoglycosides proved an effective approach in managing the patient's response to the condition. Four months later, a fever preceded the recurrence of anterior uveitis specifically in the patient's right eye. The blood tests were conclusively negative. Therefore, the medical diagnosis concluded recurrent uveitis, originating from an internal infection, and the patient's treatment involved a brief period of topically applied steroids. A six-month follow-up period shows the patient's best-corrected visual acuity is consistently 20/20 OU, with normal intraocular pressure and a calm anterior chamber (AC). This clinical report, the first of its kind, details a ring infiltrate in endogenous Klebsiella keratitis, highlighting the importance of a comprehensive workup to facilitate prompt treatment.
A less frequent presentation of herpes keratitis is herpes endotheliitis, where corneal edema and keratic precipitates are notable. Secondary or primary infection from herpes virus reactivation could result from exposure to triggers, for instance, physiologic stress or environmental factors. Laser-assisted in situ keratomileusis (LASIK) and photorefractive keratectomy (PRK), types of ocular surgery, may cause reactivation of herpes in patients, regardless of previous infection history. Presenting two cases of patients with insignificant stromal scarring, who denied any previous history of herpes, these patients developed herpes endotheliitis post-LASIK and PRK procedures. We showcase the importance of a complete preoperative evaluation, along with further investigation of any corneal irregularities, even if they appear initially to be of no concern.
